dessert. The cost is $5.00 per student, per meal.

Please list
your student’s name. Either list the dates or circle the meals that your
child wants to order, then return this form along with your check for
the meals ordered to the school office no later than the
first Monday of the month.
If you choose
to order on a weekly basis then your order and money must be in the
office no later than Monday of the week you are
ordering for. NO EXCEPTIONS.
There will be
snacks, chips, candy, soda and water available for purchase separately
on lunch days however, the listed lunch meals will only be available to
those that have pre-ordered.
